INTRODUCTION: Mid-regional proadrenomedullin (MR-proADM) reflects the adrenomedullin level, which has vasodilatory activity, decreases endothelial permeability, and downregulates proinflammatory cytokines. Sepsis diagnosis in these patients is difficult, and MR-proADM is a widely studied sepsis biomarker. This study evaluates MR-proADM levels during the resuscitation phase, considering the potential influence of haemodynamic changes and its usefulness for the early sepsis detection in burn patients. METHODS: A prospective observational study performed in the Critical Burn Unit. Demographic data, burn characteristics, comorbidities, prognostic/severity scales, and haemodynamic parameters were collected. The resuscitation protocol guided by diuresis, transpulmonary thermodilution, and lactate levels was followed. Blood samples were collected at various time points for biomarker measurement. Biomarker levels, including MR-proADM, C-reactive protein, and procalcitonin were measured during the resuscitation phase and septic episodes. RESULTS: Twenty-seven patients were included, with a mean age of 51 years, a mean total body surface area burn of 41.8%, a mean Abbreviated Burn Severity Index of 9.7, and a mean Baux score of 92. MR-proADM levels were elevated on admission (0.9 ± 0.5 nmol/l) and continued to increase slightly during the resuscitation phase (2.4 ± 2.2 nmol/l). Haemodynamic changes during resuscitation did not significantly affect MR-proADM levels. Twelve of the 27 patients developed sepsis, whose MR-proADM levels were significantly elevated on the day of clinical diagnosis (3.91 ± 2.99 nmol/l) and even the day before (2.57 ± 3.37). Higher MR-proADM levels were associated with greater severity as measured by the Sequential Organ Failure Assessment score. The mean MR-proadrenomedullin values during resuscitation in the patients who died was 3.51 ± 2.30 nmol/l, whereas in the survivors it was 1.28 ± 1.10 nmol/l (p = 0.0001). CONCLUSION: MR-proadrenomedullin values are elevated after thermal injury but are not affected by haemodynamic changes. During septic episodes in burn patients, MR-proADM rises early (the day before sepsis diagnosis). Higher levels of MR-proADM are associated with greater organ dysfunction and mortality.

INTRODUCTION: The association between viral infections and pulmonary exacerbations in children with cystic fibrosis (cwCF) is well established. However, the question of whether cwCF are at a higher risk of COVID-19 or its adverse consequences remains controversial. METHODS: We conducted an observational, multicenter, cross-sectional study of cwCF infected by severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) between March 2020 and June 2022, (first to sixth COVID-19 pandemic waves) in Spain. The study aimed to describe patients' basal characteristics, SARS-CoV-2 clinical manifestations and outcomes, and whether there were differences across the pandemic waves. RESULTS: During study time, 351 SARS-CoV2 infections were reported among 341 cwCF. Median age was 8.5 years (range 0-17) and 51% were female. Cases were unevenly distributed across the pandemic, with most cases (82%) clustered between November 2021 and June 2022 (sixth wave, also known as Omicron Wave due to the higher prevalence of this strain in that period in Spain). Most cwCF were asymptomatic (24.8%) or presented with mild Covid-19 symptoms (72.9%). Among symptomatic, most prevalent symptoms were fever (62%) and increased cough (53%). Infection occurring along the sixth wave was the only independent risk factor for being symptomatic. Just eight cwCF needed hospital admission. No multisystem inflammatory syndrome, persisting symptoms, long-term sequelae, or deaths were reported. CONCLUSIONS: Spanish current data indicate that cwCF do not experience higher risks of SARS-CoV-2 infection nor worse health outcomes or sequelae. Changes in patients' basal characteristics, clinical courses, and outcomes were detected across waves. While the pandemic continues, a worldwide monitoring of COVID-19 in pediatric CF patients is needed.

MATERIAL AND METHODS: A cross-sectional descriptive study composed of 125 people diagnosed with schizophrenia was performed. Patients were assessed with the Communication Skills Questionnaire (CSQ), the Gambrill and Richey Assertiveness Inventory (GR), the Global Assessment of Functioning Scale (GAF) and the Global Assessment of Social Functioning Scale (SOFAS), Social Functioning Scale (SFS), Life Skills Profile (LSP) scale and the Clinical Global Impression scale for Schizophrenia (CGI-S). RESULTS: SOFAS, LSP and GR are related to each of the subscales and total scores of symptoms (p<0.05-0.001). The multiple regressions show that SOFAS and GR explained 59% of the total symptoms. SOFAS and GR, accounting for 65% of the variance, explain positive symptoms. GR and SOFAS explained 34% of the variance of negative symptoms. SOFAS, CSQ and LSP, accounting for 20% of the variance, explain depressive symptoms. SOFAS explained 46% of the variance of cognitive symptoms. CONCLUSIONS: Our findings suggest the usefulness of social functioning assessment in the explanation of clinical symptoms in people with schizophrenia. Moreover, our results point out that not only negative and cognitive symptoms, but also positive and depressive symptoms, should be taken into account in the rehabilitation process in order to improve patient adaptation in the community.

Introducción: El objetivo del presente estudio fue evaluar la relación entre el funcionamiento social y los síntomas en las personas esquizofrénicas, analizando la influencia del funcionamiento social global y los aspectos específicos del funcionamiento social, la asertividad y las competencias de comunicación en la explicación del tipo de síntomas. Material y métodos: Se realizó un estudio descriptivo transversal integrado por 125 personas con diagnóstico de esquizofrenia. Se evaluó a los pacientes utilizando Communication Skills Questionnaire (CSQ), Gambrill and Richey Assertiveness Inventory (GR), Global Assessment of Functioning Scale (GAF) y Global Assessment of Social Functioning Scale (SOFAS), Social Functioning Scale (SFS), Life Skills Profile (LSP) scale y Clinical Global Impression scale for Schizophrenia (CGI-S). Resultados: SOFAS, LSP y GR están relacionadas con cada una de las sub-escalas y puntuaciones totales de los síntomas (p<0,05-0,001). Las regresiones múltiples muestran que SOFAS y GR justifican el 59% de los síntomas totales. SOFAS y GR, que representan el 65% de la varianza, explican los síntomas positivos. GR y SOFAS justificaron el 34% de la varianza de síntomas negativos. SOFAS, CSQ y LSP, que representaron el 20% de la varianza, justificaron los síntomas depresivos. SOFAS justificó el 46% de la varianza de los síntomas cognitivos. Conclusiones: Nuestros hallazgos sugieren la utilidad de la evaluación del funcionamiento social para explicar los síntomas clínicos de las personas esquizofrénicas. Además, nuestros resultados apuntan que, no solo deberían considerarse en el proceso de rehabilitación los síntomas negativos y cognitivos, sino también los síntomas positivos y depresivos, a fin de mejorar la adaptación del paciente dentro de la comunidad. Profile of Community Recovery Services users has changed over the years and has become more diverse. To explore the evolution of treatment needs, this study aimed to identify users' needs, from the point of view of different agents implicated in the recovery process. We explored the consistency between the agents using the focus group technique. We defined four groups (n = 58): service users, family members, professionals, and referring professionals. We pre-identified topics related to recovery, such as illness-related losses, imaginary of CRS, expectations, activities, and life goals. All agents recognised losses related to the mental illness, the need for carrying activities out of the Community Recovery Services, and for including families in the recovery process. The groups differed in some areas, such as the identification of activities that should be encouraged, or the importance of promoting vital expectations. Our findings suggest that it is important to identify the needs of different agents involved in the recovery process. There is consistency in the service users' needs, but there are some differences that need to be considered. Interventions should be personalised, covering functional, cognitive, and relational losses related to the mental illness.

The objective of the study was to test the usability of 'Feeling Master' as a psychotherapeutic interactive gaming tool with LEGO cartoon faces showing the five basic emotions, for the assessment of emotional recognition in people with schizophrenia in comparison with healthy controls, and the relationship between face affect recognition (FER), attributional style, and theory of mind (ToM), which is the ability to understand the potential mental states and intentions of others. Nineteen individuals with schizophrenia (SZ) and 17 healthy control (HC) subjects completed the 'Feeling Master' that includes five basic emotions. To assess social cognition, the group with schizophrenia was evaluated with the Personal and Situational Attribution Questionnaire (IPSAQ) for the assessment of attributional style and the Hinting Task (ToM). Patients with SZ showed significant impairments in emotion recognition and their response time appeared to be slower than the HC in the recognition of each emotion. Taking into account the impairment in the recognition of each emotion, we only found a trend toward significance in error rates on fear recognition. The correlations between correct response on the 'Feeling Master' and the hinting task appeared to be significant in the correlation of surprise and theory of mind. In conclusion, this study demonstrated that the 'Feeling Master' could be useful for the evaluation of FER in people with schizophrenia. These results sustain the notion that impairments in emotion recognition are more prevalent in people with schizophrenia and that these are related with impairment in ToM.

Women with schizophrenia have shown better social and clinical functioning than men in several previous studies. The aim of this study was to assess gender differences in social functioning of people with a diagnosis of schizophrenia who are in psychosocial rehabilitation treatment in Catalonia, using several different one-dimensional and multidimensional measures of clinical and social functioning. Gender differences were found only in several daily life activities, in which women showed better functioning: independence-competence (p = 0.006) and independence-performance (p = 0.017). No other differences between genders were observed. Our results clearly suggest that women with schizophrenia undergoing the rehabilitation process could benefit as do men from psychosocial intervention in order to improve social skills and functioning.

Introducción: El Carcinoma de lengua móvil es uno de los tumores malignos más frecuentes de la cavidad bucal. Generalmente, los pacientes son diagnosticados después de los 40 años de edad. La detección temprana de estas lesiones, evita su desarrollo hacia fases avanzadas de la enfermedad, que oscurecen su pronóstico, por medio de una atención rápida y adecuada, biopsia oportuna, y tratamiento precoz, evitando que los pacientes sean mutilados, sufran o mueran por esta causa. El objetivo del presente estudio es reportar un grupo de pacientes con esta neoplasia, su tratamiento y supervivencia. Métodos: En el presente estudio descriptivo, longitudinal y retrospectivo fueron evaluados los pacientes con diagnóstico de carcinoma de lengua móvil, tratados en el Instituto Nacional de Oncología de la Habana, entre los años 2007 y 2011. Se reporta edad, tabaquismo, alcoholismo, estadiaje, tipo de tratamiento y supervivencia. Resultados: Se registraron 68 casos, se observó un predominio del grupo de edad entre los años 55-69, con el 41.18% de los pacientes, siendo el sexo predominante el masculino, con un 79.41% del total. En la clasificación TNM, se observó que predominó de los tumores estadiados como T3 con el 35.29 %, seguidos de los T2 con el 30.88 %, la invasión ganglionar estuvo más representada por la categoría N0 con el 58.82% de los casos. El 64.71% de los pacientes estaban vivos a los 60 meses. Conclusión: El carcinoma de lengua móvil fue más frecuente en las edades entre 55-69 años y en el sexo masculino, más del 50% de los pacientes practicaban hábitos tóxicos, hubo un predominio de etapas avanzadas, la cirugía con adyuvancia postoperatoria fue la terapéutica más común.


Introduction: Carcinoma of the mobile tongue is one of the most frequent malignant tumors of the oral cavity. Generally, patients are diagnosed after 40 years of age. The early detection of these lesions prevents their development towards advanced stages of the disease, which obscure their prognosis, through rapid and adequate care, timely biopsy, and early treatment, preventing patients from being mutilated, suffering or dying from it cause. The objective of the present study is to report a group of patients with this neoplasia, its treatment and survival. Methods: In the present descriptive, longitudinal and retrospective study, patients diagnosed with mobile tongue carcinoma, treated at the National Institute of Oncology of Havana, between 2007 and 2011, were evaluated. Age, smoking, alcoholism, staging are reported, type of treatment and survival. Results: There were 68 cases, a predominance of the age group between the years 55-69 was observed, with 41.18% of the patients, being the predominant sex the masculine, with 79.41% of the total. In the TNM classification, it was observed that tumors predominated as T3 with 35.29%, followed by T2 with 30.88%, the lymph node invasion was more represented by the N0 category with 58.82% of the cases. 64.71% of the patients were alive at 60 months. Conclusion: The mobile tongue carcinoma was more frequent in the ages between 55-69 years and in the male sex, more than 50% of the patients practiced toxic habits, there was a predominance of advanced stages, the surgery with postoperative adjuvant was the therapeutic more common.

Early debridement and/or eschar removal is regarded as a significant step in the treatment of deep partial and full thickness burns. It aims to control wound bioburden and allows early wound closure by conservative treatment or skin grafting. Preservation of viable dermis accompanied by early wound closure, is regarded as a necessary step to reduce scar related complication, e.g. functional limitations and/or unaesthetic scar formation. Aside from the classical techniques of surgical excision as tangential excision for eschar removal, hydro-surgery, maggot therapy, laser, enzymatic debridement have been described as additional techniques in the burn surgeon's armamentarium. It is widely accepted that early eschar removal within 72h improves the outcome of burn wound treatment by reducing bacterial wound colonization, infection and length of hospital stay. In contrast, the right technique for eschar removal is still a matter of debate. There is increasing evidence that enzymatic debridement is a powerful tool to remove eschar in burn wounds, reducing blood loss, the need for autologous skin grafting and the number of wounds requiring surgical excision. In order to assess the role and clinical advantages of enzymatic debridement by a mixture of proteolytic enzymes enriched in Bromelain (Nexobrid®) beyond the scope of the literature and in view of users' experience, a European Consensus Meeting was scheduled. The aim was to provide statements for application, based on the mutual experience of applying enzymatic debridement in more than 500 adult and pediatric patients by the consensus panelists. Issues to be addressed were: indications, pain management and anesthesia, timing of application, technique of application, after-intervention care, skin grafting after enzymatic debridement, blood loss, training strategies and learning curve and areas of future research needs. Sixty-eight (68) consensus statements were provided for the use of enzymatic debridement. The degree of consensus was remarkably high, with a unanimous consensus in 88.2% of statements, and lowest degree of consensus of 70% in only 3 statements. This consensus document may serve as preliminary guideline for the use of enzymatic debridement with user-oriented recommendations until further evidence and systematic guidelines are available.

BACKGROUND: Excessive bleeding is a major concern in scar debridement and grafting procedures. TT-173 is a new topical hemostatic agent based on recombinant human tissue factor that has shown promising results in patients who underwent tooth extraction. EHTIC study sought to evaluate the efficacy and safety of TT-173 to reduce the bleeding in donor sites of skin grafting procedures. METHODS: EHTIC study was a phase II, randomized, parallel, double blind, placebo controlled trial. Patients received TT-173 (n=38) or placebo (n=33) sprayed over donor site after graft harvest. Time to hemostasis and incidence of adverse events were recorded. Systemic absorption of the product and its immunogenicity were also measured during the follow up of the subjects. RESULTS: Treatment with TT-173 significantly reduced the bleeding time from 7 to 3min (Log-Rank p<0.0001). Moreover, bleeding stopped within the 10min of evaluation period in all the patients that received TT-173. In contrast, 24.24% of patients from placebo group required additional measures to arrest hemorrhage (Fisher p=0.0013). Product related adverse events, systemic absorption into blood stream, interferences with the healing of the donor site or immunogenic reaction against TT-173 were not observed. CONCLUSION: The new hemostatic agent TT-173 has proven efficacious and safe to reduce the bleeding from donor site. This study paves the way for further investigation of the product as topical hemostatic treatment in plastic surgery and other surgical indications.

Introducción: La roncopatía es un problema de salud actual, frecuente en la población adulta por su incidencia y repercusión negativa sobre la calidad de vida de los pacientes afectados. Objetivos: Conocer la incidencia, los síntomas, los factores predisponentes, las enfermedades asociadas más frecuentes y evaluar los resultados del tratamiento quirúrgico de esta afección en la población sometida a estudio. Métodos: Se realizó un estudio descriptivo prospectivo transversal. Se estudiaron 21 pacientes, 14 (66,6 por ciento) del sexo masculino y 7 (33,4 por ciento) del femenino, atendidos en el servicio de Otorrinolaringología del Hospital Universitario General Calixto García Íñiguez durante el período comprendido de diciembre de 2008 a diciembre de 2009. Resultados. El 33 por ciento de los pacientes presentaron síndrome de apnea del sueño, más trastornos diurnos como hipersomnolencia diurna y cefalea matinal y en algunos casos popliglobulia e hipertensión arterial. Los factores predisponentes más frecuentes fueron la obesidad y la obstrucción orofaríngea. A 19 (90,47 por ciento) de los pacientes con roncopatía se les realizó tratamiento quirúrgico y evolucionaron satisfactoriamente a la semana y al mes de operados. Conclusiones: Los síntomas más frecuentes en los pacientes roncadores fueron la hipersonmolencia diurna, los ahogos nocturnos y la cefalea matinal. Los factores de riesgo más frecuentes fueron la obesidad y la obstrucción orofaríngea


Introduction: The snore disease is a current health problem frequent in adult population due to its incidence and negative repercussion on the quality of life of involved patients. Objectives: To know the incidence, symptoms, predisposing factors, the more frequent associated diseases and also to assess the results of surgical treatment of this affection in the study population. Methods: A cross-sectional and descriptive study was conducted. Twenty one patients were studied, 14 (66,6 percent) of male sex and 7 (33,4 percent) of female sex, seen in the Otorhinolaryngology Service of the "General Calixto GarcÝa I±iguez " University Hospital from December, 2008 to December, 2009. Results: The 33 percent of patients had sleep apneic syndrome plus diurnal disorders like the diurnal hypersomnia and morning headache and in some cases popligulia and arterial high blood pressure. The more frequent risk factors were obesity and oropharyngeal obstruction. A total of 19 patients (90,47 percent) were surgically treated with a satisfactory evolution at one week and at one month after surgery. Conclusions: The more frequent symptoms in snoring patients were the diurnal hypersomnia, nocturnal breathlessness and morning headache. The more frequent risk factors were obesity and oropharyngeal obstruction

RESUMO

INTRODUCCIÓN. Las enfermedades mamarias son un problema de salud también en los países subdesarrollados. Ello motivó la creación del Servicio de Mastología en el Centro Nacional de Oncología de Luanda (Angola), en el año 2007. La presente investigación tuvo como objetivo describir la actividad quirúrgica de este centro durante el año 2007. MÉTODOS. Se realizó un estudio transversal descriptivo que incluyó a los 183 pacientes sometidos a algún tipo de cirugía mamaria en el Servicio de Mastología en el Centro Nacional de Oncología de Luanda durante ese período. Los datos fueron recogidos en un formulario confeccionado por los autores y se procesaron mediante el programa estadístico SPSS (versión 13.0). La información fue resumida en frecuencias absolutas y porcentajes. RESULTADOS. En los pacientes intervenidos quirúrgicamente predominaron las enfermedades benignas sobre las malignas, con alta incidencia de fibroadenomas gigantes. La tumorectomía fue el tipo de cirugía más empleada en las enfermedades benignas, acompañadas de técnicas de cirugía plástica en pacientes con fibroadenoma gigante. Todos los pacientes con tumores malignos requirieron cirugías radicales en correspondencia con la estadificación tardía que predominó en esta serie de casos. El índice de complicaciones quirúrgicas fue bajo. Las enfermedades de la mama constituyen un problema de salud en la población estudiada. El diagnóstico tardío de tumores en etapas avanzadas determina el tratamiento quirúrgico radical sobre el conservador(AU)


INTRODUCTION. Breast diseases are also a health problem in developing countries. Hence, the creation of the Mastology Service in the National Center of Oncology of Luanda, Angola in 2007. The aim of present paper was to describe the surgical activity of this institution during 2007. METHODS. A descriptive and cross-sectional study was conducted including 183 patients undergoing some type of breast surgery in above mentioned Service during that period. Data were registered in a form designed by authors and processed using the SPSS statistical program (version 13.0). Information was summarized in absolute frequencies and percentages. RESULTS. In patients operated on there was predominance of benign diseases and plastic surgery techniques in those patients presenting with giant fibroadenoma. All patients with malignant tumors underwent radical surgeries according the late staging present in this series of cases. The rate of surgical complications was low. NS. Breast diseases are a health problem in study population. Late diagnosis of tumors in advanced stages determines the radical surgical treatment above the conservative one(AU)

Introducción: La roncopatía es un problema de salud actual, frecuente en la población adulta por su incidencia y repercusión negativa sobre la calidad de vida de los pacientes afectados. Objetivos: Conocer la incidencia, los síntomas, los factores predisponentes, las enfermedades asociadas más frecuentes y evaluar los resultados del tratamiento quirúrgico de esta afección en la población sometida a estudio. Métodos: Se realizó un estudio descriptivo prospectivo transversal. Se estudiaron 21 pacientes, 14 (66,6 por ciento) del sexo masculino y 7 (33,4 por ciento) del femenino, atendidos en el servicio de Otorrinolaringología del Hospital Universitario General Calixto García Íñiguez durante el período comprendido de diciembre de 2008 a diciembre de 2009. Resultados. El 33 por ciento de los pacientes presentaron síndrome de apnea del sueño, más trastornos diurnos como hipersomnolencia diurna y cefalea matinal y en algunos casos popliglobulia e hipertensión arterial. Los factores predisponentes más frecuentes fueron la obesidad y la obstrucción orofaríngea. A 19 (90,47 por ciento) de los pacientes con roncopatía se les realizó tratamiento quirúrgico y evolucionaron satisfactoriamente a la semana y al mes de operados. Conclusiones: Los síntomas más frecuentes en los pacientes roncadores fueron la hipersonmolencia diurna, los ahogos nocturnos y la cefalea matinal. Los factores de riesgo más frecuentes fueron la obesidad y la obstrucción orofaríngea(AU)
